Most people with Medicare automatically get Part A coverage without having to pay a monthly payment, called a premium. This is because they or their spouse paid Medicare taxes while they were working. A person who does not automatically get premium-free Part A may buy it if he or she meets certain requirements.

The SSA determines if an individual must pay a premium for Medicare Part A.
